Arranging your care package
Get in touch with us to arrange a home visit from one of our senior management team
A member of our management team will meet with you face to face within your home to discuss your needs, wishes, preferences and the range of services available. We will then work in partnership with you and any support networks you choose to be involved in your care, to develop a package of care that meets your individualistic needs. Within this meeting we will also conduct safety and risk assessments for the benefit of yourself and the care and support worker.
360 works with you and not for you; we can work together to conduct your person-centered care plan.
Once you are happy with the package of care and care plan identified we will select a team of Care and Support Workers to provide your person-centred care.
Our Registered Manager carefully matches clients and care workers on the following basis:
- Do we feel the client and care workers are well matched in terms of personality?
- Do the care workers have the appropriate experience and skills required?
- Are the care workers prepared to be assigned to the client on a permanent basis?
Once the care team has been identified and assigned to the client, care and support can be delivered. The Registered Manager will contact the client after the first few days of care to ensure the carers are a good match and the levels of care are meeting the client’s needs, any changes required will be conducted. Throughout the period of the care provided 360 management will continuously ensure that you are happy with the care provided by attending your home and carrying out regular reviews.
